It is mandatory to attend an enrollment session in order to be enrolled in the Student Employee Health Insurance Program (SEHP) for those represented by the Graduate Student Employee Union (GSEU).

Eligibility Appointed to at least one-half an assistantship, receive a stipend that would yield a total compensation of $4,122 or more annually, and are a domestic student or F1 or J1 visa holder.
Enrollment Eligible Domestic Students and F1 or J1 visa holders who choose the GSEU health insurance are required to enroll within 45 days of the effective date of their appointment. The effective date of coverage will be the effective date of your appointment.
If you are enrolling dependents please bring copies of the following documents to your enrollment session.
- Spouse: Marriage certificate, U. S. Social Security card
- Children: Birth certificate, U. S. Social Security card
- Spouses and children of F-1 visa holders who have F-2 visas are not eligible to receive a Social Security card, however, marriage and birth certificates are still required along with a copy of the stamped I-94 from their passports
Domestic Partner Eligibility Your domestic partner is eligible. Unmarried enrollees may cover same or opposite sex partners with whom they reside and have a committed, long-term relationship of mutual support. For more information for domestic partner eligibility and forms, click here.
F1 and J1 Visa holders: Your medical evacuation and repatriation coverage will be carried as a rider through the Student Medical Insurance Office, 223 Student Union, North Campus, or (716) 645-3036. The cost is $72.00 for the entire year is included in the health insurance charge on your tuition bill. Please pay the $72.00 on your tuition bill.
Cost of Health Insurance SUNY will cover 90% of the single enrollee cost, and 75% of the dependent cost as the employer contribution. The employee's contribution is as follows:

Reappointed Graduate and Teaching Assistants with Summer Coverage If you are a reappointed 10 Month Graduate or Teaching Assistant for the Fall 2009 semester and you continued your health insurance at the employee share for the Summer of 2009, you will automatically be re-enrolled in the Student Employee Health Insurance Plan for Fall 2009.
YOU DO NOT HAVE TO COMPLETE ANOTHER APPLICATION TO ENROLL.
Mandatory Health Insurance Fee: Once you are enrolled in SEHP, you will automatically be waived from the mandatory health insurance fee on your tuition bill.
*Please watch your paychecks in October 2009 to make sure the health insurance premiums are taken out of your paychecks.
Reappointed Graduate and Teaching Assistants without Summer Coverage If you are a reappointed 10 Month Graduate or Teaching Assistant for Fall 2009 and you did not continue your health insurance at the employee share for the Summer of 2009, you must attend an orientation session and re-enroll in GSEU Health Insurance. Please visit the GSEU orientations page to sign up to attend an orientation session or contact Human Resources at (716) 645-4486.
Mandatory Health Insurance Fee: Once you are enrolled in GSEU Health Insurance, you will automatically be waived from the mandatory health insurance fee on your tuition bill.

New Appointments Once you enroll in the Student Employee Health Insurance Program as a Graduate or Teaching Assistant, we will inform the Student Medical Insurance Office of your enrollment and waive you out of the mandatory program. Please pay your tuition bill in full minus the mandatory health insurance fee, which is $1,581.00 for domestic students for the entire year and $994.75 for international students for the full year.
Re-Appointed and enrolled in Health Insurance during Summer 2009 Reports are sent to the Student Medical Insurance Office to waiver you out of the mandatory program. Your new appointment paperwork must be submitted to Human Resource Services in order for you to be on this list. Please pay your tuition bill in full minus the mandatory health insurance fee. The cost for domestic students is $1,581.00 for the entire year. International students coverage for the for the spring and summer sessions is $94.75.
Health insurance through another source Please provide proof of your health insurance coverage through another source, such as spouse or parent, to the Student Medical Insurance Office at 223 Student Union, North Campus or visit their website. Once you present this proof, which should be a copy of your health insurance policy or identification card, they will waive the student mandatory health insurance from your tuition bill. It is not necessary to contact Human Resources for this situation.
